Why is the item that I would like to select from the menu unavailable or grayed out? |

Answer:
Reference Manager is task specific. For example, if you have your cursor in the upper window in the reference list, you will not be able to select "Generate from a Reference List" from the Tools->Bibliography menu. This is because you are actually in the individual reference editing mode.
To make this selection available you would simply move your cursor to the lower window. This act tells Reference Manager that you are done editing and ready to generate a reference list.
